Why titration matters
GLP-1 receptor agonists cause more GI side effects (nausea, constipation, diarrhea) when started at higher doses. The standard titration schedules (small starting dose, escalating every 4 weeks) let the gut and brain adapt gradually. Many patients who can't tolerate the standard schedule do well on a slowed schedule (5-6 weeks between escalations).
Maintenance vs target dose
The maximum trial-tested dose (Wegovy 2.4mg, Zepbound 15mg) is not always the right maintenance dose. Many patients respond well at intermediate doses (Wegovy 1.7mg, Zepbound 7.5mg or 10mg) and stay there to balance efficacy and side effects. Maintenance dose is a conversation between you and your prescriber.
